Office Based Surgery & Anesthesia

Office Based Anesthesia and Podiatry - Benefits

Podiatric health is an important component of general health. By utilizing office based anesthesia, Dr. Silvers, Dr. Lloyd and Dr. Witt can perform some procedures in the office. Benefits of office based procedures are:

  • Your procedure will be done in the office where you are familiar and comfortable

  • A lighter sedation is often administered which can reduce the recovery period

  • Shorter waiting times than hospital based procedures

  • Less paperwork and administrative overhead than hospital procedures

Sedation and Anesthesia - An Effective Method

Modern techniques of sedation and anesthesia are safe and the patient is relaxed and comfortable. Most patients remember very little about the procedure and recovery is generally quick. Along with your podiatrist, a board certified anesthesiologist will administer the anesthesia and be present for the duration of the procedure.

After the patient is evaluated, the anesthesiologist will decide on the best method to ensure patient comfort. Surgeries that are short and less invasive (as most office based procedures are) often use Monitored Anesthesia Care (MAC). IV sedation is used to assure that the sedatives and painkillers take effect rapidly and the surgical site is prepped with local anesthetic.

After discussing your preference and comfort level, the anesthesiologist will either keep you awake and comfortable or administer light to moderate sedation. Discomfort will be addressed by either administering deeper sedation or the surgeon can increase the amount of local anesthetic.

Monitoring the Patient

The patient is carefully monitored during the duration of the procedure. Specially designed monitors allow the anesthesiologist to assess patient vital signs continuously. Some of these are

  • Blood pressure monitor

  • SPO2 - measures concentration of oxygen in the blood

  • EKG - heart monitor

  • CO2 monitor - monitors patient breathing

The patient is surrounded by a team of specialists that collaborate to ensure patient safety, comfort, and a stress free experience.

Surgery

The following instructions are meant to serve as a general guideline of instructions most patients should follow to ensure a successful surgery. Every patient is contacted for phone consultation the day before your procedure by your anesthesiologist. Your physician will give you a set of guidelines specific to you and your medical history and health. During your phone call, please feel free to address any questions or concerns. Texas Star Anesthesia Management is dedicated to helping the patient understand their medical care. It is very important that patients follow preoperative rules, or scheduled surgery may have to be postponed. Your medical team will give you instructions prior to surgery.

Before Surgery

  • An empty stomach is very important. Therefore patients should not eat or drink for at least 8 hours before surgery. Do not drink or eat anything.

  • Patients should not drink or smoke for 24 hours before or after surgery. Alcohol and tobacco can adversely affect anesthesia and other medication during surgery and cause complications.

  • Patients are advised to bring all medications to the preoperative consult with the surgical team. They will advise and guide you about what medications you may take prior to surgery.

  • You cannot safely drive after surgery that requires anesthetics. You have to bring a responsible person to drive you home.

Post Surgery

Our anesthesiologist will closely monitor patients as they regain consciousness.

  • Family members will be notified when they can join the patient

  • Once fully awake and the patients vital signs are stable, the patient is discharged

  • Patients should expect drowsiness post surgery and will need assistance. There should be an adult family member or friend to care for the patient overnight.

  • Patients cannot drive or operate heavy machinery for a minimum 24 hours after surgery. While the patient may think they feel fine, their reflexes are in an altered state.

  • Some patients may need to stay overnight for observation or further treatment.

  • If a patient has unexpected pain, abnormal bleeding, a high temperature they should seek immediate medical assistance. For medical emergencies, please dial 9-1-1 or go to the nearest Emergency Room.

Frequently Asked Questions

  • Is it safe? - Yes. Sedation and anesthesia have a very strong record of safety.

  • Is the anesthesiologist certified? - After completing medical school and residency, an anesthesiologist must complete a rigorous process of board certification. They are also required to undergo continuing education courses annually to ensure they are up to date on the latest developments in the field.

  • Are there side effects? - Yes there are, but a vast majority of the patients have little or no side effects. Common side effects are nausea, vomiting, lightheadedness, stomach aches, and headaches. Discuss these with your podiatrist and anesthesiologist.

  • Can I drive home after being administered sedatives or anesthesia? - After you have undergone a procedure involving sedatives and/or anesthetics you will be monitored at the facility to ensure full post op recovery. Though you may not feel the effects of anesthesia, there are residual effects that may not be apparent. Patients are advised not to operate a vehicle and/or heavy machinery for a 24 hour period after anesthesia or sedation. This is for your own safety.

  • Is it important that I disclose my entire medical history prior to any procedure? - Absolutely. Podiatrists and anesthesiologists are medical professionals and their main purpose is to treat your symptoms so that you regain your health. Your medical history is confidential and is only disclosed to others with your permission. Treatment will vary depending on your state of health and knowing your health history is an important part of the treatment.
